Complete the Application Form and read and agree to the Terms and Conditions
Return to CYQ via email or print and send to:
Code of Practice, CYQ, 112 Great Russell Street, London WC1B 3NQ

Pay invoice. The cost is £480 (plus VAT) per site visit. The number of visits required is dependant upon the size of your organisation
Contact by a Code of Practice Evaluator to agree a date and time for your site visit
Agree date for site visit
Evaluation visit takes place
Receive final report from CYQ within 10 working days

If your Club/Centre is non-compliant:

Code of Practice Evaluator decides achieving compliance requires desk-based actions or a follow-up visit (for multi-site operators this also initiates further visits to sites selected randomly by CYQ)
A Club/Centre will not be deemed compliant until all actions have been satisfactorily completed
The date of compliance wil be backdated to the initial site visit, irrespective of when actions are completed

If your Club/Centre is compliant:

CYQ updates database
CYQ records date of approval to initiate bi-annual reminder for future approvals
CYQ informs FIA who issue FIA Code of Practice compliance plaque
